Before you embark on your fitness journey with our trainers, we require all new clients to complete two essential forms: the Physical Activity Readiness Questionnaire (PAR-Q) and the Personal Trainer Waiver and Release of Liability Form.

Athlete Needs Analysis Form

At MetaMorph Performance, we've developed a comprehensive Athlete Needs Analysis Form to assess the specific requirements of athletes across various sports. We use this tool to gather essential information about each athlete's personal details, sport-specific demands, physical profile, performance metrics, injury history, training background, and goals.


By systematically collecting this data, we can develop tailored training programs that address the unique strengths and weaknesses of each athlete we work with. We believe that the insights gained from this analysis not only enhance performance but also contribute significantly to injury prevention and overall athlete development.
